Who is Eligible?

To qualify for the TennCare Diaper Program, the following criteria must be met:

• The child must be a TennCare or CoverKids member under 2 years of age.

• If your child is a newborn without a card, you can use the mother’s pharmacy ID card or Social Security number.

• The form must be completed by a legal guardian.

Eligible families can receive up to 100 diapers every 30 days or 200 diapers every 60 days for children under 2 years old. Please complete a separate form for each eligible child to get started.   Click below to get started.
